Stomach Bug vs. Stomach Flu: What's the Difference?

At EZMed Urgent Care, we frequently see patients with gastrointestinal illnesses and dehydration. Understanding the difference between a stomach bug and stomach flu can help you know when it's time to seek medical attention.

What Is a Stomach Bug?

"Stomach bug" is a general term used to describe gastroenteritis, an infection or irritation of the stomach and intestines.

A stomach bug can be caused by:

  • Viruses
  • Bacteria
  • Parasites
  • Food poisoning

Common symptoms include:

  • Nausea
  • Vomiting
  • Diarrhea
  • Stomach cramps
  • Low-grade fever
  • Fatigue

Most stomach bugs are contagious and can spread through close contact, contaminated food, or contaminated surfaces.

What Is the Stomach Flu?

The term "stomach flu" is somewhat misleading because it is not actually influenza (the flu).

The stomach flu usually refers to viral gastroenteritis, most commonly caused by viruses such as:

  • Norovirus
  • Rotavirus
  • Adenovirus
  • Astrovirus

Unlike influenza, which primarily affects the respiratory system, stomach flu affects the digestive tract.

Stomach Bug vs. Stomach Flu

Stomach Bug Stomach Flu
General term for gastroenteritis Usually refers specifically to viral gastroenteritis
Can be caused by viruses, bacteria, parasites, or food poisoning Usually caused by viruses
May require different treatments depending on the cause Usually treated with supportive care
Symptoms vary depending on the infection Commonly causes vomiting and diarrhea

Is It Food Poisoning or a Stomach Virus?

Many people mistake food poisoning for a stomach virus.

Food poisoning often:

  • Begins suddenly
  • Occurs after eating contaminated food
  • May affect multiple people who ate the same meal

A stomach virus often:

  • Develops gradually
  • Spreads from person to person
  • May circulate through households, schools, workplaces, or daycares

Because symptoms overlap, it can be difficult to tell the difference without a medical evaluation.

How Long Does a Stomach Bug Last?

Most viral stomach illnesses improve within 1 to 3 days, though some symptoms can last longer.

You may be contagious even after symptoms improve, especially with norovirus.

Signs of Dehydration

One of the biggest concerns with vomiting and diarrhea is dehydration.

Watch for:

  • Dry mouth
  • Dizziness
  • Dark urine
  • Decreased urination
  • Fatigue
  • Rapid heart rate

Children, older adults, and individuals with chronic medical conditions may be at higher risk for dehydration.

When Should You See a Medical Provider?

You should seek medical attention if you experience:

  • Severe abdominal pain
  • Persistent vomiting
  • Bloody diarrhea
  • High fever
  • Signs of dehydration
  • Symptoms lasting more than a few days
  • Inability to keep fluids down

A healthcare provider can help determine whether your symptoms are due to a virus, food poisoning, bacterial infection, or another medical condition.
